How We Can Help Your Medical Practice

The Advancing Clinical Excellence Network is a member of the CMS Network of Quality Improvement and Innovation Contractors, a federal initiative that funds efforts such as ours to help medical practices improve quality and performance.

Our network of 12 organizations, including Altarum, will help providers in seven states reduce clinical administrative burdens so they can focus on delivering the best possible care to their patients.

This work will begin in early 2020 pending a contract award from CMS, for which our network has applied.*

Interventions that Strengthen Your Practice

The programs focuses on improving outcomes in four areas: behavioral health and opioids, patient safety, chronic disease self-management, and care transitions.

Our clinical interventions advance these aims with curricula, protocols, workflow aides, and associated measures and process-improvement strategies customized to fit your unique goals.

  • Patient medication adherence
  • Managing chronic conditions, including diabetes and hypertension

Who Is Eligible for Support?

All clinicians—physicians, physician assistants, nurse practitioners, registered nurses, and others—are eligible to participate. We work with independent providers, rural practices, large practices, and health systems.

There is no fee to participate, but you must practice in Florida, Illinois, Indiana, Kentucky, Michigan, New York, or Ohio.


What Is Required of Providers?

Practices are required to submit data each month. Participants who submit data will also benefit from feedback from our experts on ways to optimize their practice.
