Putting Care at the Center is the annual conference of The National Center for Complex Health and Social Needs. This year’s in-person event will take place September 21-23, 2022, in Sacramento, CA. Putting Care at the Center is an opportunity for the national complex care community to learn, network, and create a shared agenda for the growing field of complex care.
Jennie Chin Hansen, a member of Altarum’s Board of Trustees, is this year’s keynote speaker. Jennie is a trailblazing nurse who pioneered the Program of All Inclusive Care to the Elderly (PACE) model and a nationally recognized leader in geriatrics and complex care for older adults.
Join Altarum in the Beehive! The Beehive is the conference’s innovation and semi-structured networking space. The goal of the Beehive at Putting Care at the Center is to share emerging knowledge, experience, strategy, and challenges with and among attendees, and facilitate new partnerships and collaborations. On Friday, September 23, Christine Stanik and Sarah Slocum will facilitate “Measuring culture change to capture person-centered care and QOL progress.” The Systems Change Tracking Tool (SCTT) can identify and measure progress towards the uptake of person-centered care practices in congregate care communities. Participants will learn how SCTT results, in combination with the CMS prioritization matrix, can be used to hone in on immediately actionable and impactful areas for quality improvement.
