At the On Aging 2022 conference, Anne Montgomery, a program director in Altarum’s Delivery System Transformation practice area, and Morgan Perry, a public health analyst, will lead a workshop on “Advancing Residents’ Emotional Security: Pushing Back on Social Isolation in Nursing Homes.” The workshop will take place on Tuesday, April 12 at 4:30 pm CT.

Hosted by the America Society on Aging (ASA), the annual event is the largest multidisciplinary conference on aging held in the United States and runs April 11–14 in New Orleans, Louisiana. Conference content is curated to align with ASA’s strategic priority areas—justice and equity, innovation and social impact, health and well-being, economic security, and ageism and culture. The theme for 2022 is “Advancing Economic Security.” Altarum’s workshop will be featured in the “Lifelong Learning, Engagement, and Society track.”

Altarum’s Systems Change Tracking Tool (SCTT) captures person-centered care practices to use in quality-of-life (QOL) initiatives within Quality Assurance Performance Improvement (QAPI) projects. The SCTT and accompanying prioritization matrix identify actionable areas for needed quality improvement, while also matching other key facility priorities. For (mostly) Medicaid residents nearing the end of life, emotional security is tightly linked to getting person-centered care—and being able to measure the results.

Altarum’s workshop will cover how the SCCT was employed to track the implementation of comprehensive culture change in six Michigan nursing homes. Based on the results of this project, attendees will learn how to use Altarum’s SCTT and QOL prioritization matrix and how to collect and integrate resident input in the execution of a QAPI QOL project. They will also be able to explain the value of increasing facility resources for programming that prioritizes resident choice and experience.

Anne Montgomery will participate in an additional symposium on Monday, April 11th from 10:00–11:30 am CT. She will join fellow National Coalition on Care Coordination members Robyn Golden and Bonnie Ewald of Rush University’s Center for Health and Social Care Integration for “Bringing Care into the Community: Innovations and Policies to Support Older Adults as Needs Evolve.” This session will describe innovations that link care in the community for older adults to existing supports and provide complex care—when needed. Recent examples of enhanced service coordination include providing long-term services and supports within affordable housing, and embedding PACE within a housing site. The panelists will also distill recent policy developments and outstanding policy needs.
