Overview
Altarum has received funding from the states of Michigan, Tennessee, and Kentucky to work directly with nursing homes that are shifting from an institutional medical model to a vibrant person-centered living community. Alongside nationally known culture change experts, we provide education on specific practices, data and feedback on progress, and technical assistance that enables settings to conduct their own ongoing quality improvement efforts.
Our Approach
We work with education partners, like The Eden Alternative® and The Green House Project®, to bring intensive training and coaching to each home, while Altarum staff provide mentorship and education on data use, quality improvement methods, and how to measure key metrics for maximum impact. Converting to person-centered care from an institutional model is hard work and we stay engaged with each long-term care provider for three years. Using evidence-informed practices to create our System Change Tracking Tool℠ (SCTT), Altarum helps providers measure their progress and focus on areas for improvement.
Results
Even during the pandemic, participating nursing homes continued to make progress and many said their knowledge of person-centered practices helped them better respond to residents’ needs during pandemic restrictions. All six participating homes in Michigan said they would do this project again and all made progress on a majority of SCTT measures. Resident lives were improved and staff experienced a more positive and uplifting work environment—a win-win!

A leader in innovating the care delivery system with health plan, state, and federal partners, Krissy is proficient in health policy implementation and analysis, with a focus on home and community-based waivers and long-term services and supports. She has an eye for identifying the unintended consequences of proposed policies and creating innovative, and realistic, recommendations for state partners. Krissy has deep experience in providing technical assistance, designing and implementing action-oriented and outcomes-based learning systems, and catalyzing behavior change through thoughtful and strategic training. Passionate about improving the care delivery system, she has supported states and communities in enhancing the infrastructure, training, and experiences for direct service workers, including those who self-direct. Krissy holds a Master of Public Health degree from The George Washington University School of Public Health and Health Sciences.
